Based on my real experience (too many):

Case Study One
Natural birth - One baby came out at 2250hr and other reported at 2300hr. It is still a twin but big different in bazi.

Case Study Two
C-section birth - On the 4 February 2011, one baby at 1231hr and the other at 1234hr. Timing looks same. However, both are totally different zodiac. Very rare.
